VoIP Conference Calling – What Internet system provides maximum concurrent users?

I try to establish a permanent conference Calling on the website with users in America, Europe and West Africa. West African People may be limited by the bandwidth, the only voice. Other users should be able to use video and voice. My limited research shows SKYPE own can cause callers to the Conference of the four at once. Is this the best option available? Do you have Yahoo or MSN or any other VoIP system offers a better set up? FOUR users will be the best out of a conference via Internet.
